If you are a New Patient, please download and fill out the all three New Patient Forms, and either email the completed forms to [email protected] or bring the completed forms to your appointment.  If you would rather fill them out in person, please arrive at least 15 minutes prior to your appointment time to complete your paperwork.

*Please note our 24 Hour Cancellation/Rescheduling Policy: We understand that unavoidable circumstances arise and appointments have to be cancelled or rescheduled.  Remember, cancellation and/or rescheduling must be made 24 hours prior to your appointment.  Please call us at (424) 265-1815 to cancel or reschedule your appointment if you are unable to do this through our online booking system.  Failure to give 24 hours notice will incur a $50 fee.  Late arrivals will still be charged full rate even though the session is shortened.
